Immunization Requirements & TB Testing
Salisbury University requires all students to complete mandatory immunization and TB screening/testing requirements to help maintain a safe and healthy campus community.

Requirements, Recommendations and Deadlines
Required Immunizations
- Measles-Mumps-Rubella (MMR): Two doses given on or after first birthday or copy of blood test showing immunity with positive IgG titers to Measles (rubeola), Rubella and Mumps.
- Tetanus-Diphtheria-Pertussis (Tdap): One dose given at age 11 or older.
- Meningococcal (Meningitis) ACWY (Menveo, MenQuadfi, or Menactra): given ≥ 16 years of age is required by Maryland law for students living in on-campus housing. This vaccine can be waived by completing a meningitis waiver form found on the Student Health Portal (hyperlink to myhealth.salisbury.edu)
- Tuberculosis (TB) Exposure Risk Screening Questionnaire: Must be completed in the Student Health Portal (hyperlink to myhealth.salisbury.edu). If TB risk factors are present, you will be required to have further testing.
Submission Deadlines
All immunization documentation must be submitted upon enrollment.
- July 31 for fall enrollment
- January 15 for spring enrollment
Failure to submit required documentation by the deadline will result in delays in course registration and a $100 late fee.

- Varicella vaccine (chicken pox)
- Hepatitis A vaccine
- Hepatitis B vaccine
- HPV vaccine (Gardasil 9)
- Polio vaccine
- Annual COVID-19 vaccine
- Seasonal influenza vaccine (flu shot)

Collect Your Documents
Gather your required records and documentation before beginning the submission process.
Sign In
Access the portal using your university credentials to securely upload and manage your information.
Submit Information
Enter your information, upload supporting files, and complete any required questionnaires or forms.
Immunization Review Process
The Health Center team reviews all submissions for required immunizations.
Reviews may take between 5 to 7 business days before they are registered to student accounts. If you have questions about your immunization compliance status after this time period has elapsed, please contact us at 410-543-6262.
Health Center Vaccines
Getting a vaccine on campus
- The Health Center offers MMR, Tdap, Meningitis ACWY, seasonal influenza, and Hepatitis B. Please call 410-543-6262 to schedule.
- In the fall, Salisbury University partners with Acme Pharmacy offering multiple flu and COVID vaccine clinics at convenient locations on campus. More details to come via campus email.
